Frozen Specialties, Inc. - Archbold, OH: On or about May 27, 2015, the employer did not ensure that energy control procedures were utilized by authorized employees who unjammed the Morrison auger at the Eagle line. c. Frozen Specialties, Inc. - Archbold, OH: On or about April 28, 2015, the employer did not ensure that energy control procedures were utilized by authorized employees who made adjustments to track the South Line Doughboy conveyor belt.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a. Frozen Specialties, Inc. - Archbold, OH: On or about May 27, 2015, the employer did not ensure that employees were protected from hazardous energy sources while unjamming boxes from the Sprinter machine's conveying line. The machine was not shut down and energy isolating devices were not used to when employees were exposed to pinch points and rotating part hazards to remove jammed boxes. b. Frozen Specialties, Inc. - Archbold, OH: On or about May 27, 2015, the employer did not ensure that employees were protected from hazardous energy sources while unjamming product from the Morrison screw auger on the Eagle line. Employees were exposed to amputation injury in that the machine was not shut down and energy isolating devices were not used when unjamming product. c. Frozen Specialties, Inc. - Archbold, OH: On or about April 28, 2015, the employer did not ensure that employees were protected from hazardous energy sources while making an adjustment to track the South Line Doughboy conveyor belt. Employees were exposed to injury from in-running nip points in that the machine was not shut down and energy isolating devices were not used when removing a guard to make the adjustment.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a. Frozen Specialties, Inc. - Archbold, OH: On or about April 28, 2015, the employer did not ensure that employees who service equipment such as, but not limited to, the Sprinter machine, Eagle line, and conveyors, were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees that unjammed and made adjustments to the machines were not trained to isolate all energy sources during those tasks.
