29 CFR 1910.179(n)(3)(iii)(a): During hoisting care was not taken to ensure there is no sudden acceleration or deceleration of the moving load: (a) Facility, Hardening Department, 726 Bell Avenue, Carnegie, PA: On or about April 15, 2015 during the charging of the #4 furnace a sudden deceleration of the load through the refractory donut cased an employee to suffer an injury when their hand was caught between the falling load and the rotator plate.
