29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean. a) Production Area: The employer failed to ensure that the floor, piping, walls and equipment remained free from the accumulation of combustible wood dust. Employees were exposed to a fire/explosion hazard while performing duties involving the cutting of wood pieces for the manufacture of wood shelves. A sample taken on April 23, 2015 was determined to be a CLASS II combustible dust.

On or about May, 20, 2015 the electrical disconnect on the Holzma CNC; model HPL 11 was not locked out prior to an employee attempting to remove a jam and/or miss fed piece of laminate.

Employees were allowed to utilized compressed air for cleaning at an operating pressure above 80 PSI while performing saw operations for the manufacture of wood shelves.

Employees were exposed to a fire/explosion hazard while sawing, sanding, and performing various activities for the manufacture of wood shelves. A sample taken on April 23, 2015 was determined to be a Division I CLASS II explosive combustible dust. a) Florescent lighting in the area was not explosion proof lighting.
