29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    a) Back wall of production area:  On or about March 1, 2018,  and at times prior, the employer exposed employees to fire hazards in that the exit door was equipped with a slide lock and a lock on the door handle which did not  allow employees quick egress in case of an emergency.    b) Shipping department:  On or about March 1, 2018, and at times prior, the employer exposed employees to fire hazards in that the exit door was equipped with a slide lock and a lock on the door handle which did not allow employees quick egress in case of an emergency.

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:    a) Throughout establishment:  On or about March 1, 2018, and at times prior, the employer exposed employees to electrical hazards, in that a periodic inspection was not provided for all authorized employees on all machines that require lockout tagout procedures, such as but not limited to, the Vantage machine, Radius machine and Bander machine.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.     a) Throughout establishment:  On or about March 1, 2018, and at times prior, the employer exposed employees to electrical hazards, in that lockout tagout training for authorized employees was not provided for all employees that assist with maintenance on machines, such as but not limited to, the Vantage machine, Radius machine and Bander machine.
