OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: caught between hazards caused by the movement of the well drilling equipment when assembling the drill stem.   On or about April 21, 2015, employees at the Laura 3408 Lease/Well Number 1-26H 2L were performing oil well drilling operations. The floor hand working the drill floor drill rig was exposed to caught-between hazards from the jerk chain and the head ache pole.  Among other methods, some feasible and acceptable abatement methods to correct this hazard may include but are not limited to the following:       1.  Develop, implement and enforce procedures requiring positive assurance from the floor hands that they are clear of the equipment prior to the driller putting a strain on the jerk chain.         2.  Ensure that signals, signs and/or other types of communication between supervisors, personnel and other involved persons are agreed upon and fully understood and utilized prior to initiation of operations as indicated in Paragraph 12.3 of the American Petroleum Institute Recommended Practice 54.
