29 CFR 1910.1018(e)(2):  Initial monitoring.  Each employer who has a workplace or work operation covered by this standard shall monitor each such workplace and work operation to accurately determine the airborne concentration of inorganic arsenic to which employees may be exposed:   a.  On or about April 25-27, 2015, at the job site where bridge blasting was being performed, the employer did not conduct initial employee monitoring to determine the airborne concentration of inorganic arsenic.

On or about April 23, 2015, at the job site where bridge activity was being performed, the two areas of vehicle approach on the roadway were not properly posted with legible traffic signs at the points of hazard.

Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers shall conform to Part VI of the Manual on Uniform Traffic Control Devices, (1988 Edition, Revision 3 or the Millennium Edition), which are incorporated by reference in ��1926.200(g)(2):   a.  On or about April 23, 2015, at the job site where bridge activity was being performed, the two flaggers were not using hand-signaling devices, such as STOP/SLOW paddles, lights, and red flags to control vehicular traffic through the temporary traffic control zone.
