29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as used as a means of access.  a. The employees were accessing the catenary scaffold by climbing up under the bridge, the break in elevation was 26 inches where employees were observed entering and exiting the scaffold, thereby exposing employees to a fall hazard of 26 inches.

29 CFR 1926.451(g)(2): Effective September 2, 1997, the employer shall have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds. Employers are required to provide fall protection for employees erecting or dismantling supported scaffolds where the installation and use of such protection is feasible and does not create a greater hazard.  a. The employees erecting the catenary scaffold were working without fall protection and exposed to a 9 foot fall over rock and a large fall hazard over moving water.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. Employees were observed working on a bridge where they were leaning over the guard rail to provide material to employees working underneath the bridge, thereby exposing employees to a fall hazard of greater than 18 feet.
