29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: (a) At the 3665 Cherryvale, Appleton jobsite; a 3M 4000 Reusable Roof Anchor had been installed using four 16 penny nails instead of the ten 16d 3" double heads in order to obtain the 5,000 pound support capacity.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: (a) At the 3665 Cherryvale, Appleton jobsite; the employer did not provide training on the correct procedures for erecting the 3M 4000 Reusable Roof Anchor. The roof anchor had been installed using four 16 penny nails instead of the ten 16d 3" double heads in order to obtain the 5,000 pound support capacity.
