29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) On 4/24/15, an employee performing roofing activities was exposed to a fall hazard of approximately 10 feet from the roof of the house at 8 S. Queen St. in Shippensburg, PA.  Employee(s) were not using any acceptable means of fall protection.    Abatement certification required within 10 days after the abatement date.  The certification shall include a statement that the abatement is complete, date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      a) On 4/24/15, at 6 S. Queen St. in Shippensburg, employee(s) were using a folded Blue Ribbon step ladder to gain access to the scaffold platform at the front of the house.  The ladder did not extend above the platform height and no grasping device was provided.    b) On 4/24/15, at 6 S. Queen St. in Shippensburg, employee(s) were using a folded Werner step ladder to gain access to the roof of the house from the scaffold platform at the front of the house.   The ladder did not extend three feet above the roof edge, nor was a grasping device provided.    Abatement certification required within 10 days after the abatement date.  The certification shall include a statement that the abatement is complete, date and method of abatement, and state that employees and their representatives were informed of this abatement.

Queen St. in Shippensburg, employee(s) were using a folded Blue Ribbon step ladder to gain access to the scaffold platform at the front of the house.  The legs were not extended as the ladder is designed to be used.  b) On 4/24/15, at 6 S. Queen St. in Shippensburg, employee(s) were using a folded Werner step ladder to gain access to the roof of the house from the scaffold platform at the front of the house.
