29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof:  a) Jobsite, 1502 Frankford Avenue, Philadelphia - Fire extinguishing equipment was not available on the roof, where employees were performing hot roofing using a propane torch.  Observed on or about 4/16/15.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone (i.e. without the warning line system) is permitted:    a) Jobsite, 1502 Frankford Avenue, Philadelphia - Employees performing hot roof work on a flat roof were exposed to fall hazards of approximately thirty (30) feet without the use of conventional or alternative methods of fall protection.  Observed on or about 4/16/15.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Jobsite, 1502 Frankford Avenue, Philadelphia -  The ladder did not extend at least three (3) feet above the upper landing surface, exposing employees to a fall hazard during access/egress.  Employees were performing hot roof work, approximately thirty (30) feet above ground level.  Observed on or about 4/16/15.
