29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59): a. Jobsite located at 2618 East York Street, Philadelphia, PA 19125-3648 - The employer did not provide employees with information and training on the hazardous chemicals in the work area, including but not limited to, styrene, etc. contained in CoREZYN® COR-GRIP PA-160-PMMN (Interplastic Corporation, 5019 Hunt Street, Pryor, OK 74361-9539), a general purpose adhesive bonding putty. Employees applied CoREZYN® COR-GRIP PA-160-PMMN as a general purpose adhesive bonding on a flat roof. Observed on or About April 23, 2015

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501: a. Jobsite located at 2618 East York Street, Philadelphia, PA 19125-3648 - Employees were exposed to fall hazards. The employer did not provide fall protection to two employees who were exposed to falls from unprotected sides and edges of a flat roof that was approximately 30 feet above the ground. Two employees were using spreaders to apply CoREZYN® COR-GRIP PA-160-PMMN, a general purpose adhesive bonding putty, onto the unprotected edges of a flat roof of a three story building without conventional fall protection systems (i.e. personal fall arrest systems, guardrail systems, or safety net systems). Observed on or about April 23, 2015. Modelist, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 1020331, Citation Number 1, Item Number 1a, issued on 03/25/2015, with respect to a workplace located at 43 West Johnson Avenue, Philadelphia, PA 19144. The final order date for the citation was on April 20, 2015.
