29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Worksite - On or about April 28, 2015, employees were exposed to overhead struck-by hazards from falling or flying debris such as, but not limited to, old shingles, pieces of insulation, and pieces of wood due to not wearing protective safety hard hats.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a) Worksite - On or about April 28, 2015, employees were exposed to struck-by hazards to the eyes due to using tools to scrape objects such as, but not limited to, old shingles off the roof without wearing safety glasses, and then throwing the debris off of the roof where other employees were working on the ground below the roof without any of these employees wearing safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Roof - On or about April 28, 2015, employees were exposed to fall hazards when working on a 6 on 12 pitched roof of approximately 12 feet high without adequate fall protection.    Bluman Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 935845, citation number 2, item number 1 and was affirmed as a final order on 11/1/13, with respect to a workplace located at 26 Sonya Place, Niskayuna, NY 12309.    Bluman Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313763500, citation number 1, item number 3 and was affirmed as a final order on 7/11/11, with respect to a workplace located at 176 Washington Avenue Extension, Albany, NY 12205.
