29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. (a) Hafa Day Flea Market Building, Ground Level Facing Hafa Adai Theater: On about April 22, 2015, the employer failed to provide ground fault circuit interrupters for employee that used HILTI TE500 AVR Hammer Drill outdoors exposed to rain. The drill was powered through an extension cord missing a ground prong. The cord was plugged into an exterior outlet on the building. Employee was exposed to the hazards of electrical shock.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: (a) Hafa Day Flea Market Building, Ground Level Facing Hafa Adai Theater: On about April 22, 2015, there was no grounding prong on the extension cord that powered a 110 VAC HILTI TE500 AVR Hammer Drill. The drill was powered through an extension cord missing the prong. The cord was plugged into an exterior outlet on the building. Employee was exposed to the hazards of electrical shock.

(a) Hafa Day Flea Market Building, Second Floor Room G16: On about April 22, 2015, 110 VAC yellow extension cord was not protected from damage as it lay on the concrete floor and a metal door frame. The cord was exposed to being pinched between the door and the metal frame. Employees were exposed to the hazards of electrical shock and burn.

(a) Hafa Day Flea Market Building, Second Floor Room G16: On about April 22, 2015, the employer did not provide ladders or steps for employees to access a red color metal frame platform located approximately 28 inches above the concrete floor. The employee was possibly exposed to fall hazards.
