29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not also effectively closed: (a) Theater Ground Level Behind The Stage, Near East Exit and Entry Area: On or about March 31, 2015, 110 VAC two outlet junction box was not provided with a face cover plate and the exposed terminals were energized. The bottom of the box was approximately 24 inches above the floor. Employee was exposed to possible accidental contact with exposed live parts and receive an electrical burn and shock.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws: (a) Theater Ground Level Behind The Stage, Near East Exit and Entry Area: On or about March 31, 2015, three (3) wires were pulled away from the plug on an extension cord that powered a 110 VAC portable drill. The plug end was approximately 24 inches above the floor. Employee was exposed to possible accidental contact with exposed live parts and receive an electrical burn and shock.
