29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:  a- 8 Monterey Bay Road, Block 176.04, Lot 5, Tinton Falls NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  An employee at this site were exposed to hazards including, but not limited to fall hazards while performing roof sheathing installation while wearing a full body harness without a lanyard attached.  As such this employee was exposed to fall hazards in excess of 12 feet to the ground surface below.  Condition noted on or about 04/30/15.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a- 8 Monterey Bay Road, Block 176.04, Lot 5, Tinton Falls NJ: An employee, wearing a full body harness but without a lanyard attached, was exposed to fall hazards in excess of 12 feet to the ground surface below when performing roof sheathing installation and other related activities from a new residential structure's roof with a pitch of approximately 4 on 12.  This employee had not received fall protection training nor fall hazard recognition training from the company at the time of the worksite inspection.  Condition noted on or about 04/30/15.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 8 Monterey Bay Road, Block 176.04, Lot 5, Tinton Falls NJ: An employee, wearing a full body harness but without a lanyard attached, was exposed to fall hazards in excess of 12 feet to the ground surface below when performing roof sheathing installation and other related activities from a new residential structure's roof with a pitch of approximately 4 on 12.  Condition observed on or about 04/30/15.   High Quality Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1060058, citation number 1, item number 1 and was affirmed as a final order on 03/27/15, with respect to a workplace located at 7 Herbert Avenue Pehala Park, Long Beach Township NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
