29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) On or about 05-04-15 at Hwy 441 and Coldsprings Road, an employee was utilizing the lifting-eye on a concrete form as an anchor point of a personal fall arrest system at approximately 15 feet 6 inches above the ground level. The lifting eye used in the fall protection system was attached to unsecured formwork.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about 05-04-15 at Hwy 441 and Coldsprings Road, an employee was utilizing a personal fall arrest system with a 6 ft. lanyard anchored approximately 1 ft. above the walking/working surface and approximately 15.5 ft. above surface level. The personal fall arrest system with shock absorbing lanyard was not installed to prevent contact with the lower surface level.
