29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about May 5, 2015 the employer did not assure the #1 punch deriveter machine was guarded. Employees operating the press were exposed to amputation and crushing hazards while removing rivets from vehicle brakes shoes. FleetPride Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 761172, citation number 1, item number 3 and was affirmed as a final order on October 4, 2013 with respect to a workplace located at 2831 2nd Avenue, Birmingham, AL.
