29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Employees were exposed to amputation and laceration hazards due to guarding deficiencies in the following instances: 1) In the West Service Shop Area, employee(s) engaged in automotive repair and fabrication operations were exposed to amputation and laceration hazards in that a Hines DL 500 Driveline Balancer was not guarded. 2) In the East Service Shop Area, employee(s) engaged in automotive repair and fabrication operations were exposed to amputation and laceration hazards in that a Delta/Rockwell drill press chuck was not guarded. 3) In the West Service Shop Area, employee(s) engaged in automotive repair and fabrication operations were exposed to amputation and laceration hazards in that a Dayton 17" drill press chuck was not guarded.

29 CFR 1904.41(a)(2): Annual electronic submission of information from OSHA Form 300 Log of Work-Related Injuries and Illnesses and OSHA Form 301 Injury and Illness Incident Report by establishments with 100 or more employees in designated industries. If your establishment had 100 or more employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix B to subpart E of this part, then you must electronically submit information from OSHA Forms 300 and 301 to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.
