29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: a) 1100 Market Street, Philadelphia, PA 19107: Employees demolishing a free standing column were exposed to falls of more than 50ft. Employees were standing outside of the basket while tied to the anchorage point inside the aerial lift. Conditions were observed on or about May 5, 2015.
