29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about 5/5/15 and 5/7/15 in the bending machines area at 6144 Highway 341 South in Fort Valley, GA: Where authorized and affected employees were setting up, adjusting, troubleshooting, repairing and servicing malfunctioning equipment no periodic inspection was performed annually on the energy control procedures for the bending equipment.  One employee was injured on 5/5/15 while trying to correct the malfunctioning bending machine.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used:  On or about 5/7/15 in the welding production areas at 6144 Highway 341 South in Fort Valley, GA: Employees operating powered industrial tucks and walking in the welding area were not protected from the arc rays with a protective screen or appropriate eye protection.  The Blue Bird Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.252(b)(2)(iii), which was contained in OSHA inspection number 315736843, citation number 1, item number 2 and was affirmed as a final order on 3/14/2012, with respect to a workplace located at 402 Blue Bird Blvd in Fort Valley, GA 31030.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about 5/7/2015 in the bending machine area at 6144 Highway 341 South in Fort Valley, GA: Electrical wiring at up to 440 volts on the bending machines being operated and maintained by the employees were not provided with strain relief to protect the wiring hanging from the machines electrical connectors and components on the equipment.
