On 5/8/2015, employees from Cheltenham Construction Services, Inc. were installing siding on a residential building located on lot 98 in Wilmer Valley Subdivision in Wentzville MO.  They were working on a pump jack scaffolding that had poles erected on footings which were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement.  All four of the poles supporting the pump jack scaffolding were not on level and sound footings to prevent displacement.
