29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: On or about May 7, 2015, at the above addressed job site, an employee was exposed to a fall hazard of about 15 feet while painting on a roof while using a lifeline that was tied in a knot to an anchorage that had sharp edges. PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED.
