On or about May 27, 2021, on the west side of the building, employees were working from an aerial lift without the use of harness and lanyard attached to the boom or basket, exposed to an 11 feet fall hazard. Dacosta Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1236974, citation number 1, item number 1, and was affirmed as a final order on June 28, 2017, with respect to a workplace located at 788 Las Olas Blvd. Fort Lauderdale, FL 33301.
