Mikron Valve and Manufacturing, Inc., Production Area - On or about May 8, 2015, three compressed gas cylinders of argon, were free standing and not properly secured to prevent falling.

Mikron Valve and Manufacturing, Inc. - On or about May 8, 2015, and times prior thereto: Employees worked with or around hazardous chemicals including, but not limited to, Carbon Monoxide, Ethyl Alcohol, and various coolants and lubricants during production operations. The employer had not developed and implemented a written Hazard Communication Program.
