29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 employees in the Machine Shop were required to operate the following machinery without guarding: A.) employees were observed operating a Mazak Quick Turn Nexus 300II with the safety interlocks disengaged. The disengaged interlocks can cause employees to become injured by nip points, rotating parts/tools or flying chips/fluid. B.) employees were observed operating a Mazak Quick Turn 20N lathe with the safety interlocks disengaged. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 employees in the Machine Shop were required to operate the following machinery without guarding: A.) employees were observed working near and walking by a Joraensen A-891974-1 conveyor system. The front cover was missing exposing the conveyor's moving parts. The conveyor is approximately 50 inches from ground level. The missing guards exposes employees to moving and rotating parts. B.) employees were observed working near and walking by a Joraensen A-891974-1 conveyor system. On the West side of the machine was an exposed rotating shaft. The rotating shaft exposes employees to nip points.

29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work a) Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 the Jet Electric grinder model JBG-8A stock #577102 Serial #439206 in the South East of the grinder cell was missing work rests on both the left and the rght side of the machine. The missing work rests can cause material to be pulled into the machine potentially causing the wheel to shatter. b) a) Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 the red bench grinder East of the grinder cell was missing a work rest on the right side of the machine. The missing work rests can cause material to be pulled into the machine potentially causing the wheel to shatter.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: a) Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 the Jet Electric grinder model JBG-8A stock #577102 Serial #439206 in the South East of the grinder cell was missing tongue guards on both the left and the right side of the machine. The missing guard can cause material or portions of the wheel to be directed toward the operator causing injury. b) a) Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 the red bench grinder East of the grinder cell was missing a tongue guard on the right wheel. 29 CFR 1910.215(a)(2)(i): Safety guards on all operations where the work provides a suitable measure of protection to the operator, were not so constructed that the spindle end, nut, and outer flange were not exposed: a) Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 the red bench grinder East of the grinder cell was missing a guard. The spindle end, nut and outer flange were exposed during operations of the bench grinder. The missing wheel guard exposes employees to rotating parts and eliminates protection if the grinding wheel breaks.

Pressures of greater than 30 p.s.i can cause chips, material or air to potentially cause injuries to employees skin, tissues and eyes. b) Mikron Valve and Manufacturer, Inc. / 1259 West 11th Street, Erie, PA: On or about May 8, 2015 the air line used without a relief device at the assembly test and valve area was measured at approximately 50 psi. Pressures of greater than 30 p.s.i can cause chips, material or air to potentially cause injuries to employees skin, tissues and eyes.
