OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and blunt force trauma hazards from the lack of operator restraint devices on powered industrial trucks.: A.) Mikron Valve & manufacturer, Inc - On or about 2/11/20, and at times prior thereto, a Mitsubishi forklift was being operated without an operator seatbelt. The forklift was utilized to maneuver items that included but were not limited to pallets of parts in the warehouse area.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were maintained in a legible condition: A.) Mikron Valve & manufacturer, Inc - On or about 2/11/20, and at times prior thereto, a Mitsubishi forklift was being operated with an illegible forklift plate. The plate was illegible because it was not written in English. Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement
