OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Clarion Sintered Metals, Inc. / 3472 Montmorenci Road, Ridgway, PA: On or about May 7, 2015 and times prior thereto, employees were exposed to struck by and crushing hazards from the Accurate Industries stationary compactor Model #44XHD serial #11637 in that the employer did not ensure emergency stopping features were within three feet of the material feed area. An employee suffered an injury at the compactor area while the ram was cycling. At the time of the incident the only E-stop available was located approximately 103 inches (8.58 feet) from the edge of the material feed area. The employee suffered severe crushing/struck by injuries to their right leg which then resulted in an amputation.

OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Clarion Sintered Metals, Inc. / 3472 Montmorenci Road, Ridgway, PA: On or about May 27, 2015 and times prior thereto, employees were exposed to crushing and amputation injuries during the operation of powdered metal (P/M) presses provided with inadequate guards at the point of operation in that hands or other body parts can enter into the point of operation by reaching through, over, under or around the guard in place. Powdered metal presses with inadequate point of operation include, but is not limited to, the presses Autoline #2. The presses did not have an identifying tag but were identified by Clarion Sintered Metals number #1223 and #1224. Additionally, Autoline 18 Gasbarre 60 ton serial #20061324, Autoline 3 Gasbarre 100 ton serial # 20021201, Autoline 21 Gasbarre 45 ton serial #99325 also was inadequately guarded at the point of operation. b) Clarion Sintered Metals, Inc. / 3472 Montmorenci Road, Ridgway, PA: On or about May 27, 2015 and times prior thereto, employees were exposed to crushing and amputation injuries at other hazard areas of the powdered metal (P/M) presses. Other hazard areas include moving and rotating parts in the moving mechanical section below the point of operation and the mechanical components behind the P/M presses. P/M presses inadequately guarded at other hazard areas includes, but is not limited to, powdered metal presses in the back room, Autoline #1, Autoline #2, Autoline #4, Autoline #18, Autoline #20 and Autoline #21.
