29 CFR 1926.501(b)(13):  "Residential construction."  Each employee engaged in residential construction activities 6 feet   (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502:    a) On or about May 21, 2015, at a jobsite located at 20630 Orchard Court, Frankfort, IL. - an employee working on a roof with a 6:12 pitch was not protected from falls created by the presence of unprotected sides and edges, thereby exposing the employee to fall hazards of approximately twelve (12) to twenty (21) feet.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:  a) On or about May 21, 2015, at a jobsite located at 20630 Orchard Court, Frankfort, IL. - the employer did not provide a training program to ensure employees performing roofing activities were trained to recognize fall hazards in their work environment, thereby exposing the employee to fall hazards of approximately twelve (12) to twenty (21) feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step:    a) On or about May 21, 2015, at a jobsite located at 20630 Orchard Court, Frankfort, IL. - employees performing carpentry  operations on the second floor were using the top step rung of the step ladder to access their work zone,  thereby exposing the employees to fall hazards of approximately ten (10) feet.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired:  a) On or about May 21, 2015, at a jobsite located at 20630 Orchard Court, Frankfort, IL. - employees performing carpentry operations were using a portable ladder to access the upper level (second floor) that was not secured to prevent kicking out,  thereby exposing the employees to fall hazards of approximately ten (10) feet.
