On or about August 16, 2018, at the above address, an employee was required to operate a powered industrial vehicle and had not been provided with training, including but not limited to formal instruction, practical training and an evaluation of the operators performance, thereby exposing employees to struck by and crushing hazards.

On or about August 16, 2018 at the above addressed jobsite, an employee performing window installation operations from a job made box on a PIV, approximately twelve (12) feet to thirteen (13) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.

On or about August 16, 2018, at the above addressed jobsite, an employee was using an unapproved attachment (a wooden job made box) on the forks of a power industrial vehicle (PIV TEREX SS-636 Turbo forklift) to hoist an employee that was installing windows, thereby exposing the employee to fall and struck by hazards.
