29 CFR 1926.501(b)(2)(ii): Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:  On or about 05-14-15 on a 54 ft. W bridge along High Street in Wrightstown, employees walking and working in the materials staging area and who where not engaged in leading edge work, were not protected with conventional fall protection equipment. Employees were exposed to falls of approximately 15 feet above surface level from:   a)  The partially guarded edges along the east and west sides of the bridge in the material staging area. Single wire ropes were installed along each side edge of the bridge.  b)  The unguarded southern edge of the material staging area adjacent to the leading edge work area. Caution tape was installed parallel to the leading edge.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface:  a)  On or about 05-14-15 on a 54 ft. W bridge along High Street in Wrightstown, employees were working at approximately 15 ft. above surface level without an adequate guardrail system on the east and west side edges of the bridge in the staging area.  A single wire rope was used at each side edge.

29 CFR 1926.502(b)(9): The employer failed to ensure that wire rope used as a top rail was flagged at 6-foot intervals with high-visibility material.  a)  On or about 05-14-15 on a 54 ft. W bridge along High Street in Wrightstown, employees were working at approximately 15 ft. above surface level using wire rope on the sides of the bridge along the materials staging area for fall protection without attaching any markings/flags.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a)  On or about 05-14-15 on a 54 ft. W bridge along High Street in Wrightstown, employees were working at heights of six (6) ft. to approximately fifteen (15) ft. above surface level each utilizing a personal fall arrest system with the body harness attachment point located below the center of the back.

29 CFR 1926.503(c)(3): 29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee.  a)  On or about 05-14-15 on a 54 ft. W bridge along High Street in Wrightstown, employees who where working on a bridge at heights of approximately 12 ft. and 15ft were not provided with retraining in fall protection safety from the employer when employees were using improperly donned body harnesses and inadequate fall protection equipment such as inadequate guardrail system on side edges and unsecured floor hole covers.

W x 4 ft. L with a surface edge six (6) ft. above the top of a bridge pier and 12 ft. above the surface level without securing the covers.
