B.) Perry Construction Group, Inc., 350 East 6th Street, Erie, PA 16507, east side / first floor of structure, on or about May 14, 2015 and times prior thereto: The Kidde dry chemical fire extinguisher (M/N: PRO 10CM-6, 460HDM-6, S/N: XH-524852) had not received its annual inspection and servicing since June 2013.
