29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel:    a) Jordan Forest Products - The employer failed to ensure employees operating forklift trucks maintain a clear view of the path of travel, exposing employees to the hazard of being struck-by the load and/or forklift trucks. The operator of a Linde forklift (Model # H160/02; serial # E11401E00040) was moving in the forward direction with stacks of lumber that obstructed the operator's view.        On or about May 22, 2015 an employee was crushed between the carts of lumber that were being moved up to the south side #2 kiln entrance by a Linde forklift.

29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training in other unique or potentially hazardous conditions of the workplace that could affect safe operation:    (a) Southside #2 Kiln: The employer failed to ensure that a forklift operator was trained to sound the horn or utilize other means to warn personnel that stacks of lumber were being moved to the kiln entrance. The forklift operator was utilizing a Linde forklift (Model # H160/02; serial # E11401E00040) and the operator's vision was obstructed. An employee was exposed to the hazard of being struck by and/or crushed between the loads of lumber.

29 CFR 1910.1200(h)(1): Employees were not provided information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area.     a) Jordan Forest Products: The employer failed to provide training for all employees exposed to the hazards of combustible dust while performing assigned duties involving pallet manufacturing and sawmill operations. Samples collected on June 05, 2015 were determined to be a CLASS II combustible dust.
