29 CFR 1910.22(a)(3):Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a) Sawmill: On July 27, 2020, and at times prior, the employer exposed employees to slip hazards in that work floors in the sawmill contained wood shavings as employees worked on and around the wood processing machinery. b) Planer Mill, Rear Exit: On July 27, 2020, and at times prior, the employer exposed employees to slip hazards in that work floors in the sawmill contained wood shavings as employees worked on and around the wood processing machinery. c) Sawmill, Entrance: On July 27, 2020, the employer exposed employees to slip hazards from roof leaks where rainwater had accumulated from a damaged ceiling as employees walked across the facility. d) Planer Mill: On July 27, 2020, the employer exposed employees to slip hazards from roof leaks where rainwater had accumulated from a damaged ceiling as employees walked across the facility.

29 CFR 1910.147(c)(4)(ii)(C):The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them. a) Sawmill: On July 27, 2020, and at times prior, the employer exposed employees to contact with hazardous energy in that the machine-specific energy control procedures do not sufficiently describe the steps for placing and removing lockout devices onto machines as employees perform maintenance and servicing activities such as changing blades, clearing jams, and cleaning lasers.
