29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On or about May 18, 2015 in building 138, employees had been directed to manually remove the timed fuze detonators for 5 inch/38 white phosphorus rounds. The employees were exposed to potential explosion hazards from the detonators, bursters and spontaneous fire hazards from the white phosphorus. No body protection or barriers were present that would prevent exposure.

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: (a) On or about May 18, 2015 in building 138, employees had been directed to manually remove the timed fuze detonators for 5 inch/38 white phosphorus rounds while wearing yellow "fuzzy duck" gloves. The gloves were not fire resistant or designated as safe for chemical usage, which exposed employees to chemical and fire hazards.
