29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.   (a)J.J. Construction, at 5100 Ronald Reagan Blvd., Johnstown, CO 80534: On and before May 28th, 2015, the exposing employer did not ensure that an employee working off of a JLG man lift wore a body belt and lanyard attached to the boom or basket.  The condition exposed an employee to fall hazard of approximately 10 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  (a)J.J. Construction, at 5100 Ronald Reagan Blvd., Johnstown, CO 80534: On and before May 28th, 2015, the exposing employer failed to ensure that employees working with an unprotected side or ledge 6 feet or more above a lower level were protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Specifically, an employee was observed working on a wooden saw horse located on a second floor balcony without any fall protective measures in place.  The condition exposed employee to a fall hazard of approximately 13 feet from the balconies edge.
