29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 2, 2015, at a job site located at US Route 30 and Route 172 bridge, Dalton, OH, during bridge restoration work including carpentry activities, the employer did not ensure employees were protected from possible head injuries by ensuring head protection was provided and worn by employees during bulkhead building operations.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: On or about June 2, 2015, at a worksite located at US Route 30 and Route 172 bridge, Dalton, OH 44618, the employer did not ensure employees accessing a mobile scaffold were provided proper access. Employees were climbing the front of the truck using the bumper, the side of the truck, and the built in ladder did function properly due to paint build up.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about June 2, 2015, at a worksite located at US Route 30 and Route 172 bridge, Dalton, OH 44618, during bridge restoration activities including but not limited to carpentry work installing containment bulkheads from a mobile scaffold, the employer did not ensure employees working from an elevated platform were protected from fall hazards approximately 12-16 feet by a fall protection system. APBN INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.451(G)(1)(VII), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313785396, CITATION NUMBER 02, ITEM NUMBER 002, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 2, 2012, WITH RESPECT TO A WORKPLACE LOCATED ON THE I75 DISALLE BRIDGE, TOLEDO, OHIO 43609.
