29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Belt and pulleys of the shaker system: On or about June 5, 2015 and at times prior thereto, the employer did not ensure each employee conducting servicing and repair work on and around the unguarded belt and pulleys affixed their own personal lockout device on the electrical cut out which exposed the employees to amputation and other injuries associated with being struck-by and caught-in moving machinery and the in-going nip point of the belt and pulley system.
