OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to explosion hazards while welding on a tank that was full of flammable material such as liquid asphalt.     a) On or about 05/03/2016 - Asphalt Plant #3, an explosion occurred on a 30,000 gallon liquid asphalt tank while an employee was welding on the hatch.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    a) On or about 05/03/2016 - Asphalt Plant #3, employees performing maintenance and servicing, (welding) did not place a lock at the control stations prior to accomplishing maintenance activities.    Good Hope Contracting Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(ii)(D), which was contained in OSHA inspection number 1068923, citation number 01, item number 001 and was affirmed as a final order on August 21, 2105, with respect to a workplace located at 624 East State Crusher Road, Falkville, AL 35622.
