29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) At the jobsite, employees were exposed to a 12' fall hazard, in that, fall protection was not used while cleaning and chalking the second floor for framing. Violation observed on or about June 9, 2015.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) At the jobsite, an employee was exposed to a 23'-4" fall hazard, in that, fall protection was not used while installing plywood on a 5.5:12 pitch roof. The lanyard was not connected to the harness worn by the employee. Violation observed on or about June 9, 2015.    b) At the jobsite, an employee was exposed to a 23'-4" fall hazard, in that, fall protection was used incorrectly while installing plywood on a 5.5:12 pitch roof. The lanyard connected to the harness was wrapped around a truss and the snaphook was connected to itself via the rope instead of an approved anchor. Violation discovered on or about June 9, 2015.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a. At the jobsite, employees were exposed to a 23'-4" fall hazard, in that, fall protection training was inadequate. The employer allowed the employees to wrap the lanyard around the truss and the snaphook was connected to itself via the rope instead of an approved anchor. Employer allowed the use of the lanyard in a manner unapproved by the manufacturer.
