29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    a) For the worksite establishment in Sago Village at 1975 Marshberry Rd., Lot 521, Trinity, FL, as observed on or about 11/19/2015, employees, employees clipped their lifeline onto their front chest strap, instead of tying off to the rear D-ring as the harness was safely designed for.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) For the worksite establishment in Sago Village at 1975 Marshberry Rd., Lot 521, Trinity, FL, as observed on or about 11/19/2015, employees were performing framing work on a two story residential house. Employees were exposed to fall hazards of 12 ft. and ¾ inches while standing on the first floor overhang roof section and 22 ft. and 5-7/8 inches while working from the second story roof of the house.    Torre Fuerte Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1068999, citation number 1, item number 2a, and was affirmed as a final order on 08/18/2015.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a) For the worksite establishment in Sago Village at 1975 Marshberry Rd., Lot 521, Trinity, FL, as observed on or about 11/19/2015, employees were not aware of the correct safe procedure of how to tie off to their harness. Instead of tying off to the rear D-ring as the harness was safely designed for, employees clipped their lifeline onto their front chest strap.  Torre Fuerte Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.503(a)(2)(ii)), which was contained in OSHA inspection number 1068999, citation number 1, item number 2b, and was affirmed as a final order on 08/18/2015.
