29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  31237 Palm Song Place. - Employees were exposed to a 10'5" fall hazard, in that, the extension ladder did not extend 3 foot above the roof edge, observed on or about 06/11/2019.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.    (a)  31237 Palm Song Place. - Employees were exposed to a 10'5" fall hazard, in that, the extension ladder utilized to ascend and descend the roof was damaged, in that the ladder was visibly misshapen and the top rung was broken, observed on or about 06/11/2019.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  31237 Palm Song Place. - Employees in the process of decking the 6 in 12 slope (vertical to horizontal) roof, were exposed to a 10 foot fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 06/11/2019.  Torre Fuerte Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1106968, citation number 2, item 1a, and was affirmed as a final order on 12/22/2015, with respect to a workplace located at 1975 Marshberry Dr., Trinity, FL.  Torre Fuerte Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1068999, citation number 1, item 2a, and was affirmed as a final order on 8/18/2015, with respect to a workplace located at 33156 Azalea Ridge Dr., Wesley Chapel, FL.
