29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate or fixture canopy. Site: 1605 Broadway New York, NY On or about 6/18/15 a) Junction boxes with energized electrical wires were missing a cover. The electrical connection on Air Handling Unit 13 was missing the faceplate. The junction boxes located on the 5th and 12th floors.
