The most recent occurrence of this took place at the new home being built at 10315 S. 124th Street, Papillion, NE where the employer had two employees delivering roof shingles to a new construction site and allowing them to use the conveyor belt system as means of access and egress without having provided handholds (rope, chains or guardrails) for the employees to hang on to while riding the conveyor.
