29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  The employer is failing to protect employees from the physical hazard of being struck-by carriage on the KVAL door machine. This was most recently documented at a worksite located at or near 5701 S 72nd St., Omaha, NE 68127 on or about December 7, 2016. The specific energy control procedure for the carriage on the KVAL door machine did not address the potential energy source of gravity. An employee was hit in the abdomen area with a carriage that fell on them which was not blocked in place to hold the weight of the carriage.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:    The employer is failing to protect employees from the hazards of continued injuries by not properly identifying injury/illness trends from the OSHA 300 log. This was most recently documented at a worksite located at or near 5701 S 72nd St., Omaha, NE 68127 on or about December 7, 2016. The employer did not ensure that the OSHA Form 300 for the calendar year of 2016 were completed as required by the regulation to include, but not limited to:    a)     Fractured Pelvis, Fractured Ribs, Separated Shoulder, Possible Bladder Puncture - Object/substance that directly injured or made person ill was not described.  b)     Puncture wound L Index Finger - Object/substance that directly injured or made person ill was not described.   Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.
