29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Irwin 3- On or about 6/3/2015, maintenance employees were able to enter the danger zone at the point of operation to check for burrs on the trim press during the operating cycle exposing them to puncture and amputation hazards.
