29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) At 3255 Thomason Avenue, Montgomery AL - AL - 1 Thermo-former on or about March 1, 2023, and at times prior thereto, employees were required to clean the oven of the AL - 1 Thermo-former without affixing a lock to the electrical energy (E1). b) At 3255 Thomason Avenue, Montgomery AL - AL - 4 Thermo-former S/N 2215 on or about March 1, 2023, and at times prior thereto, employees were required to clean the oven of the AL - 4 Thermo-former S/N 2215 without affixing a lock to the electrical energy (E1). c) At 3255 Thomason Avenue, Montgomery AL - AL - 6 Thermo-former S/N5872 on or about March 1, 2023, and at times prior thereto, employees were required to clean the oven of the AL - 6 Thermo-former S/N 2215 without affixing a lock to the electrical energy (E1). d) At 3255 Thomason Avenue, Montgomery AL - AL - 7 Thermo-former on or about March 1, 2023, and at times prior thereto, employees were required to clean the oven of the AL - 7 Thermo-former without affixing a lock to the electrical energy (E1).
