29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening.  Boat house; on or about 09 June 2015 and times prior thereto; fall protection was not provided between the boat house and the gangway connection. An opening of approximately 30 inches in width; with a height of approximately 15 feet over the water was not guarded. This condition exposed employees to a fall hazard.

New bunkhouse, boiler room; on or about 09 June 2015 and times prior thereto; emergency exit lights were inoperative.  Emergency light was damaged. This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

New bunkhouse, west end and four-plex entryway; on or about 09 June 2015 and times prior thereto; two instances were noted where exits were not posted along the exit route:        (a) New bunkhouse west end, adjacent to latrines, exit route was not marked with an exit sign.        (b) Four-plex entryway, exit route was not marked with an exit sign.  This condition exposed employees to delays in exiting the building and smoke inhalation, fire, and tripping hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.   Warehouse, work bench area; on or about 09 June 2015 and times prior thereto; the work rest on a Dayton 6 inch bench grinder, model number 4Z123G, lot number 200107 was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured approximately 1/4 of an inch. This condition exposed employees to a caught in hazard.

Old bunkhouse, kitchen area; on or about 09 June 2015 and times prior thereto; the employer did not ensure that the electrical panels were readily accessible. A microwave oven and a countertop were placed in front of the electrical panel impeding immediate access.  This condition exposed employees to an electrical shock hazard.

Warehouse, Fish Cache; on or about 09 June 2015 and times prior thereto; a 25 foot extension cord was used as a substitute for permanent wring. This condition exposed employees to electrical shock and electrical burn hazard.
