29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: Machine shop area lathe machine, on or about June 5, 2015, employees were exposed to caught-in hazards while cutting the inside diameter of a metal tube on a LeBlond Lathe (Electrical Control Serial: T3830) and having to reach inside the tube while rotating wearing a leather glove.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: Machine shop lathe area, on or about June 5, 2015, and at times prior thereto, employees were exposed to caught-in hazards while machining on a LeBlond Lathe (Electrical Control Serial: T3830) the inside diameters on 12 inch metal tubes and using a job made tool to reach inside the tube to remove metal debris while the tube was rotating.
