29 CFR 1910.147(c)(5)(ii)(C)(2): Tagout devices and their means of attachment were not substantial enough to prevent inadvertent or accidental removal. a) Building 75, Station 51 (un-numbered): On or about August 13, 2020, and at times prior thereto, employees were exposed to a caught-in hazard from the test motor not having an adequate lockout device to prevent startup of engine due to the starter switch shutoff handle being removed, allowing bypass of the lockout device. b) Building 75, Station 53: On or about August 13, 2020, and at times prior thereto, employees were exposed to a caught-in hazard from the test motor not having an adequate lockout device or tag on the fuel line shut-off valve, ensuring that the valve remain in the closed position.

29 CFR 1910.23(b)(2): Ladder rungs, steps, and cleats were not spaced less than 10 inches (25 cm) and not more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, and steps: Water Cooling Tower, near Building 151: On or about August 13, 2020, and at times prior thereto, employees were exposed to a fall hazard while ascending and descending a fixed ladder attached to the tower that had its first rung spaced above 10-14 inches from the ground level below.
