29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent as specified in paragraph (e)(3) of this section): a) LTC Building, Track #1-2 Bridge A: On or about January 29, 2016, and times prior thereto, employee(s) were exposed to a fall hazard of greater than four feet to the ground below while standing walking, and working on bridges which were not equipped with intermediate rails. b) LTC Building, Track #1-2 Bridge B: On or about January 29, 2016, and times prior thereto, employee(s) were exposed to a fall hazard of greater than four feet to the ground below while standing walking, and working on bridges which were not equipped with intermediate rails. c) LTC Building, Track #1-3 Ramp: On or about January 29, 2016, and times prior thereto, employee(s) were exposed to a fall hazard of greater than four feet to the ground below while standing, walking, and working on ramps which were not equipped with sufficient top rails or intermediate rails.

29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors: Division 8, Building 131: On or about January 28, 2016, and times prior there to, the employer did not supervise the correct use of hearing protectors for employee(s) performing FZG and TZT Testing operations who were exposed to continuous noise at a dose of 87.9% (8-Hour Time Weighted Average sound level of 85 dBA) during the 469 minute sampling period. For a full eight hour work shift this exposure is equivalent to an average sound level of 89.0 dBA. Exposure calculations include a zero increment for the 11 minutes not sampled.
